Print forms and bring with you to the Horner Center.

All athletes, including transfers, must have a physical before beginning any participation with Hanover's athletic teams.

Please use the links to download and print the appropriate paperwork. After filling out the forms, report to the Athletic Training room in the Horner Health and Recreation Center to complete the remainder of the physical.

Hanover's team physicians will administer the athletic physical once the paperwork is complete.
